اَللَّهُمَّ صَلِّ عَلَى مُحَمَّدٍ وَ آلِ مُحَمَّدٍ
allāhumma ṣalli `alā muḥammadiw wa-āli muḥammad
O Allah, send blessings upon Muhammad and the family of Muhammad

وَ ابْعَثْ ثَـوَابَهَا إِلَى قَـبْرِ
wab`ath thawābahā ilā qabr
and send its reward to the grave of
Say the name of the deceased
